OUR IMPACT
Real Support. Real Results.
When someone is navigating cancer or chronic illness, the barriers are rarely medical alone.
It’s the missed ride to treatment.
The fatigue that makes cooking impossible.
The appointment that feels overwhelming without support.
Wellness Bridge Foundation exists to close those gaps — and the impact is measurable.

Real Numbers. Real Lives.
Because of the generosity of our donors and corporate partners, we have provided:
110 rides to life-saving screenings, chemotherapy, radiation, infusions, and specialty appointments
38 nourishing meals delivered to households during treatment and recovery
20 treatment totes filled with comfort items and essentials for infusion days
Each number represents more than a service delivered. It represents a patient who made it to treatment on time.A family that did not have to choose between groceries and gas.A person who walked into an appointment supported — not alone.

A Voice From Our Community
About
My care sister didn’t just make sure I got to treatment—she stayed by my side, prayed with me, and helped me make sense of everything. On my first ride, she showed up with a treatment tote that reminded me I was seen and cared for, and when meals became hard to manage, she made sure I had food so I could focus on healing. When my benefits ended, she met with me to make sure I didn’t have a gap in my insurance. Every time I sat in that chair, she reminded me that I was not in this alone. I truly couldn’t have done this without her.

How We Create Lasting Impact
The Village Model in Action
Our coordinated Village Model addresses the non-medical drivers that most often disrupt care. Through four integrated pillars, we strengthen continuity and improve outcomes:
Access to Care
Reliable transportation to screenings, treatment, infusions, dialysis, and follow-up visits — reducing missed appointments and delays.
Nutritional Stability
Home-delivered meals that support strength and recovery during active treatment and chronic disease management.
Care Navigation & Coordination
Support that helps individuals manage appointments, understand systems, and reduce fragmentation in care.
Care Companions
Trained advocates who walk alongside patients during vulnerable moments, providing emotional support and presence.
This is not episodic assistance. It is coordinated, barrier-reducing care.

Care That Stayed
About
Wellness Bridge Foundation was there for me when my family couldn't be. From my very first Uber health ride, they "showed up" with a treatment tote and stayed by my side, offering unwavering support until my appointment. During my, surgeries they didn't just support me; they comforted my family, prayed and sat alongside us every step of the way. Their presence meeant the world to me during one of the mostchallenging times of my life

Because Someone Stepped In
About
I wanted to attend a survivor support event but didn’t have a ride or anyone to go with me. Wellness Bridge Foundation made sure I wasn’t alone and escorted me to the event. On another occasion, they also sat with me during an appointment and followed up afterward to check in and make sure I was okay.
What meant the most was that the support didn’t stop there. There were monthly check-ins to stay connected and responsive to my changing needs, which made me feel supported over time. Knowing someone was consistently there for me made a real difference.”*
