Navigating cancer or chronic illness can be overwhelming—but you don’t have to do it alone. Our Health Navigation program offers personalized guidance, compassionate support, and dedicated advocacy through trained community health workers who walk alongside individuals and families every step of the way.

 

Whether you’re newly diagnosed, deep in treatment, or managing long-term care, we are here to help you make informed decisions, access essential services, and feel confident and empowered throughout your journey.

We connect you to transportation (BridgeRide), nutrition and exercise support , mental health counseling, and more—ensuring comprehensive care is within reach.

Support for Caregivers

Caregivers are often the invisible heroes. Our Care Counseling program offers one-on-one support, caregiver support groups, and temporary care resources—ensuring caregivers feel seen, supported, and sustained.
